COVID Safety

General Rules (updated June 12, 2023)

  1. You must be fully vaccinated at least 14 days before attending your first rehearsal. “Fully vaccinated” means:
    • You have received a booster vaccination at least 14 days before the rehearsal; OR if you are not yet eligible for a booster:
      • You have received a second dose of the Pfizer or Moderna vaccines within the past five months and at least 14 days before the rehearsal; OR
      • You have received a first dose of Johnson & Johnson in the past two months and at least 14 days before the rehearsal.
      • You have received a second dose of the Pfizer or Moderna vaccines within the past five months and at least 14 days before the rehearsal; OR
      • You have received a first dose of Johnson & Johnson in the past two months and at least 14 days before the rehearsal.
      • You have received a second dose of the Pfizer or Moderna vaccines within the past five months and at least 14 days before the rehearsal; OR
      • You have received a first dose of Johnson & Johnson in the past two months and at least 14 days before the rehearsal.
      • You have received a second dose of the Pfizer or Moderna vaccines within the past five months and at least 14 days before the rehearsal; OR
      • You have received a first dose of Johnson & Johnson in the past two months and at least 14 days before the rehearsal.
      • You have received a second dose of the Pfizer or Moderna vaccines within the past five months and at least 14 days before the rehearsal; OR
      • You have received a first dose of Johnson & Johnson in the past two months and at least 14 days before the rehearsal.

      • You have received a first dose of Johnson & Johnson in the past two months and at least 14 days before the rehearsal.
  2. To confirm that your vaccinations are up to date, please send a screenshot of your vaccination QR code to vaccine@eastbayharmony.org. (See next section for details.)
  3. Masking is optional at rehearsals. (If you wish to wear a mask, you might want to consider getting a singer’s mask such as the Resonance Mask or the Broadway Singer’s Mask, which may permit more comfortable breathing.)
  4. You must check in at every rehearsal, so we can be sure that we have a record of your attendance for contact-tracing purposes.
  5. Any eating or drinking (apart from an occasional sip of water) must be done outside the rehearsal space.
  6. If you ever have a positive test result or diagnosis of COVID-19, you must notify a board member immediately.
  7. You must stay home whenever you have reason to believe that you will put others at risk.

Proof of Vaccination (QR code)

The state of California maintains a record of each person’s COVID-19 vaccinations, which can be accessed by scanning a personalized QR code. We require that all chorus members give us access to that official record so we can confirm that they have been vaccinated. (A printed vaccination card isn’t sufficient, because we have no way to verify that it’s genuine.)

To see your personal vaccination record, go to https://myvaccinerecord.cdph.ca.gov/. You’ll be asked to enter your first and last name, your date of birth, and your email address or cell phone number. You’ll then need to create a 4-digit PIN, check a confirmation box, and click Submit. A few moments later, you’ll receive a link by email or text.

When you click the link and enter your PIN, you’ll arrive at a display of your vaccination information and a QR code. Please take a screenshot of the QR code and send it to vaccine@eastbayharmony.org. We ask that everyone do this in advance, so that we don’t have to take time to look up people’s vaccination records at the door.

This document confirms your understanding and acceptance of our COVID-19 safety protocols. Please download it, print it out, read it carefully, initial and sign where indicated, and bring it with you to your first rehearsal.
